Plantary Gear Failure in Teralow 4:1

What would cause the main input bearing just forward of the plantary gears in a Teralow 4:1 to fail? I have experienced this problem twice now. The first time was about 2 years after the install of the 4:1 and this past Memorial Day Weekend (a little over 2 years since the last failure).

I change my fluids regularly, I don't have any driveline vibrations, I do not exceed the max RPM per the Teralow instructions, and my stock T-Case had no bearing issues the whole time it was in prior to my 4:1 kit.
