Ballistic fabrication generic winch plate. I bought this several years ago for my last project and never used it. It is dusty from sitting on the shelf, but never installed.
Ballistic has it on their site (below) for $51.99. My price is $40.
I will be in down town SLC next Wednesday evening 11/6, though the morning of Friday 11/8. I could meet you at my hotel or nearby for delivery.
